RENTAL REQUEST The
guest________________________________agrees to rent _________________________________________(name of rental unit). The term
of this rental shall be from ________________________ to _________________________. The property will be ready for occupancy
at 2:00 PM on the beginning date of the lease and must be vacated by 10:00 AM on the ending date of the lease. Terry Sadler,
owner and/or manager, is in receipt of your security deposit in the amount of $__________ and has reserved above named
home/condo in your name for the above referenced date. This reservation deposit is a security deposit only and is not applied
to the rental fee. You will receive your deposit back via U.S. Postal Service approximately two weeks after your departure
after the unit has been inspected for any unreasonable damages that may have occurred during your stay. The guest hereby agrees
to the following rules and regulations during their stay: 1.) The agreed upon rental fee for the above referenced home/condo
is $ __________________( inc. cleaning fee and 12.5% FL tax) and will be paid by personal check, traveler's check or money
order (U.S. funds only) due 14 days prior to arrival date. 2.) Owner, or manager, may inspect the home at reasonable times
and intervals or in case of an emergency. 3.) Changes in the terms stated here must be in writing on this form and signed
by both parties. 4.) The tenant agrees to abide by the rules and regulations of the condominium and homeowner's association
provided in this agreement and those posted inside unit. 5.) The owner/manager can be reached at (386) 257-2454 between 9:00
a.m. and 4:00 p.m., Monday through Friday, or at any time in case of emergency. 6.) Pets are not permitted. 7.) Guests are
personally responsible for all long distance and collect calls. Please place all calls outside Daytona area on a calling card
or on personal cell phone. 8.) Trash should be securely tied in plastic bags and placed in dumpsters or trash receptacle.
Recycling required. 9.) Please consider your neighbors. Loud noises disturb most people. Television and stereos should be
controlled for the benefit of others, especially in condo units. All disturbance calls will be monitored and management will
take appropriate action. 10.) No items will be hung over balconies in condo units, and all bulky items such as floats, etc.
must be stored on the rear balcony. 11.) A parking permit will be provided for condo units, and an additional parking permit
will be given according to the parking spaces available at that time. Please check with office if an additional permit is
needed. 12.) All unit damages should be reported immediately (Terry @ 257-2454) so that the damage can be repaired in a timely
manner. Normal wear and tear is expected from time to time and will not be taken out of your security deposit. 13.) Refunds/Cancellations
will be made provided notice of cancellation is received in writing at least 90 days prior to check-in date for stays of one
month or longer, and 30 days prior to check-in date for stays less than one month. If the required notice is not received,
refunds may only be made if the reserved unit can be leased for the same time period. Cancellation of any reservation made
less than 30 days before arrival is subject to a 10% handling charge. __________________________ _______________ (Signature
